Community Hospice of Albany
The Community Hospice offers a program of services to provide support and care for persons in the final stages of life, and for their families. We offer compassionate services that respect the dignity of those we serve.

We bring care to people wherever they may be - in their own home, or in a hospital or nursing facility where we collaborate with the facility staff to enhance the quality of life for the resident. We want to make it possible for people to be comfortable in familiar surroundings with family and friends nearby.

We support the person's loved ones, too, with education and information and personal care during this difficult time. Grief and bereavement services are available for all family members, including special programs for children of all ages.

Community Hospice offers help and comfort in seven counties in upstate New York: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene and Montgomery, serving nearly 4,000 patients each year.
